Placement

The Placement determines where the bristle will actually be drawn relative to the cursor.

With the Centered at Cursor setting, the bristle is always drawn with it's center point at the cursor:

A second bristle is added to the Brush Designer. Notice the red rectangle around the icons in the Bristles list. The dot in the center indicates that the placement of these bristles is set to Centered at Cursor.

When drawn with this placement setting, they will be painted at the same location (the cursor position), regardless of their location in the Brush Designer:
